Fill out the form and mail it to the Office for Supply Management Services (SMS) for signature at 395 Pine Tree Road, Suite 330, Ithaca, NY 14850. SMS will fax the form to Verizon Wireless Customer Service and will request an e-mail confirmation to the Cornell user.
